3.If he/she does not show you his/her professional regulation commission veterinarian I.D..
4.If he/she does not issue a vaccination certificate.
5.If their vaccination certificate issued to you are in index card or xerox form.
6.If they signed your vaccination certificate and prescription without their printed name.
